In home skilled nursing can help your child live comfortably at home, and offer guidance for parents and caregivers.
We care for children with long-term illnesses as well as those recovering from hospitalization in the comforting environment of their own home. Our Registered nurses perform regular assessments and work closely with your child's physician to create a customized home care plan to fit your child's individual needs.
An RN or LPN will come to the home and provide a variety of services:
administer medication
vital signs
wound care
Colostomy care
Patient-parent education
Also a wide range of other nursing services, depending on the care needed and the physician orders. Patients may receive multiple visits a day or just a few each week.
PRIVATE DUTY NURSING SERVICES
Continuous care defined as more then four hours but fewer than 12 hours per visit.
Our nurses take care of the patients basic daily needs and helps with chronic health problems.
PDN's aim to help consumers stay within their home and live happy, healthy and fulfilling lives.
Who Is Eligible to participate?
You must be Medicaid eligible, require continuous skilled nursing services and have a comparable institutional level of care.
